If you like, print and complete the forms, then bring them to your appointment. Information about the payment methods we accept is listed below, as well. Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our Whitehall dental office and we will gladly send or fax them to you.

Financial Information
You may pay for your dental care with cash, check, American Express, Discover, MasterCard, or Visa. Our office works with most dental insurances and electronically files claims as a courtesy to our patients. If you wish to finance your dental work, CareCredit offers low- and no-interest payment plans to suit almost every budget. Visit the CareCredit website to apply online.
